ATTESTATION INSTRUCTIONS
All attendees must complete the attestation & evaluation forms at www.opso.org/2023cme
in order to earn CME credit for participation in this program.
To sign in, please use the email address you entered when originally registering for the program.
Once you are signed in, you will see the 2023 Northwest Conference and an orange CME Credit button, click that button
Then click on Customer Attendance Confirmation
Then click the radio buttons next to each presentation that you attended. IMPORTANT, please make sure to click the SUBMIT button at the bottom of the screen. This will save your selections and apply the appropriate credit for your attendance.
You will then see an EVALUATION option for each of the presentations - please complete each evaluation and hit SUBMIT.
When all your attestations and evaluations are completed, you can download a copy of your CME certificate
If applicable, OPSO & WOMA will file your CME certificate with the AOA after the end of our on-demand access period on December 3rd, 2023.
